Caregiver Counseling

FamilyMeans is an approved provider of Family Caregiver Services - Caregiver Counseling as described in the MHCP Provider Manual.

Services are available statewide (in-person or virtual). There is no cost to your client or their family for this service covered by Elderly Waiver (S5115 TF, S5115 TF U4) or Minnesota Senior Health Options (MSHO) plans from Blue Cross and Blue Shield of Minnesota, HealthPartners, and Medica.

What We Do:

Through an assessment and planning process, we help caregivers set goals and identify ways to move forward. Caregivers gain confidence, skills, and guidance through making plans and decisions while maintaining a healthy, balanced life. FamilyMeans calls this service Caregiver Coaching & Consultation.

The caregiver consultant, or coach, helps caregivers:
